Your Appointment

What to expect on the day of your appointment:

~ We practice a locked door policy. When you arrive at Lost Lakes for your appointment, give us a call at (608)-709-5557 & we will unlock the door and welcome you in.

~ If you appear to have any symptoms of Covid-19, we will ask you to leave & reschedule.

~ The counter staff will get you checked in. If it is your first time at the shop, we will ask for your ID (valid government-issued ID, Driver’s license, passport, or military identification) We will also have you sign a Deposit Agreement and Consent Form.  

~ Please wash your hands before your tattoo begins.

~ Only bring essential items with you to your appointment.

~ If you would like to bring a guest. Check with your Artist first. Our space is limited.

~When bringing food, please store meals in your vehicle. You are welcome to bring in snacks and candy.

~Water & other non-alcoholic beverages are allowed. 

~ All artists accept cash. Most artists have the option to pay via Paypal, Venmo, or credit/debit card. The credit card machine has a 3% convenience fee. 

•  Please, let your Artist know as soon as possible if you need to reschedule. If you cannot reach them call or e-mail the shop.
• The shop has a strict policy that prohibits anyone under the age of 18 from entering. It is illegal to tattoo a minor in the state of Wisconsin.
